Job Title: Direct Support Professional

We are seeking a dedicated and compassionate Direct Support Professional to join our team at Friendship Community Care. As a Direct Support Professional, you will play a vital role in providing support and care to individuals with disabilities, enabling them to live independently and participate fully in their communities.

Job Summary:

The Direct Support Professional will work closely with our interdisciplinary team to deliver high-quality support services, promoting the health, safety, and well-being of our clients. This role requires a strong commitment to providing person-centered care, respecting the autonomy and dignity of each individual.

Key Responsibilities:
  • Assist clients with daily living skills, including personal care, meal preparation, and household management.
  • Provide transportation to and from activities and appointments, ensuring safe and reliable transportation.
  • Maintain accurate and detailed records of client progress, including behavioral reports, incident reports, and progress notes.
  • Collaborate with the interdisciplinary team to develop and implement individualized plans of care, ensuring that each client's needs are met.
  • Participate in ongoing training and professional development to enhance skills and knowledge in areas such as medication management, first aid, and crisis intervention.
  • Contribute to a positive and respectful work environment, promoting a culture of teamwork and open communication.
Requirements:
  • High school diploma or equivalent required; two+ years of experience working with individuals with disabilities preferred.
  • Valid driver's license and reliable transportation required.
  • Ability to lift up to 100 pounds and perform physical tasks as needed.
  • Strong communication and interpersonal skills, with the ability to work effectively with diverse populations.
  • Ability to maintain confidentiality and handle sensitive information with discretion.
