About the Role

We are seeking a highly skilled and experienced Registered Nurse RN to fill the role of Care Coordinator in our cancer care team. As a key member of our multidisciplinary team, you will play a vital role in ensuring seamless continuity of care across all settings.

**Job Summary:**
This is an exciting opportunity to join Orlando Health as a Care Coordinator Nurse RN, working closely with patients, families, and healthcare teams to provide comprehensive discharge planning and follow-up care. You will be responsible for identifying patients who would benefit from care coordination services, assessing their risk factors, and developing effective plans to ensure timely and safe transitions between care settings.

Key Responsibilities:
  • Lead continuity and consistency of care across inpatient, emergency, and ambulatory settings.
  • Develop effective working relationships with Patient and Family Counselors/Social Workers, Utilization Review Nurses, and other interdisciplinary team members.
  • Monitor progress towards discharge plans daily, adjusting as needed to prioritize high-risk patients.
  • Educate patients and families about evidence-based standards of care, self-management strategies, and health system navigation.
  • Identify support needs and develop action plans to promote patient independence and well-being.
  • Ensure timely access to prescriptions, durable medical equipment, and other essential services.
  • Collaborate with IT resources to facilitate registry reporting and maintain specified patient populations.
  • Participate in clinical outcome measurement to identify population health strategies.
Requirements:
  • Bachelor of Science in Nursing (BSN) degree (effective July 1, 2024).
  • Current license as an RN in the State of Florida.
  • Three years of direct clinical experience, preferably in oncology or related field.
  • Excellent analytical, communication, and team-building skills.
  • Awareness of medical, legal, and regulatory issues relevant to healthcare.
