Key Responsibilities- Develop and implement personalized care plans for residents, ensuring their physical, emotional, and social needs are met.
- Provide direct patient care, including administering medications, performing medical procedures, and monitoring vital signs.
- Collaborate with interdisciplinary teams to coordinate care and ensure seamless transitions.
- Communicate effectively with residents, families, and healthcare professionals to ensure informed decision-making.
- Participate in quality improvement initiatives and contribute to the development of policies and procedures.
- Current RN licensure in the state of Ohio.
- Valid CPR certification.
- Experience in long-term care or a related field.
- Knowledge of laws, regulations, and guidelines related to long-term care.
