Job Summary

Tufts Medicine is seeking a highly skilled Pharmacist - Home Infusion to join our team. As a key member of our healthcare team, you will be responsible for providing exceptional pharmacy services to patients in a home infusion setting.

Key Responsibilities
  • Direct and coordinate clinical activities of the assigned pharmacy service area.
  • Prepare, compound, and dispense medications to patients in accordance with state and federal laws.
  • Review and accurately interpret physician orders, checking for appropriateness, clarity, accuracy, and potential drug interactions.
  • Collaborate with physicians and other healthcare professionals to ensure optimal patient care.
  • Provide accurate, timely, and effective drug information to patients, families, and healthcare professionals.
  • Participate in multidisciplinary team meetings to improve medication reconciliation and patient outcomes.
  • Develop and implement treatment guidelines, protocols, and clinical pathways to optimize patient care.
  • Verify coverage eligibility and benefits to determine the best route to obtain prior authorizations.
Requirements
  • Bachelor's degree in Pharmacy.
  • Pharmacist License.
  • One (1) year of related experience.
  • Doctor of Pharmacy (PharmD) preferred.
  • Board Certification preferred.
  • Four (4) years of related experience in clinical pharmacy expertise with direct patient care experience required.
