We are seeking a skilled RN Infusion Liaison to provide direct nursing services related to infusion therapy for patients in various settings. The primary focus of this position will be to provide education to patients discharging from area hospitals, while secondary responsibilities will involve providing infusion nursing care in the home or clinic.
Key Responsibilities- Provide infusion nursing care in various settings, including hospitals, homes, and clinics.
- Collaborate with healthcare providers, patients, and caregivers to ensure seamless care coordination.
- Develop and implement individualized care plans to meet the unique needs of patients.
- Conduct patient education and training on infusion therapy and related topics.
- Participate in ongoing quality improvement initiatives to enhance patient outcomes and care delivery.
- Current Oregon Registered Nurse License.
- National Provider BLS - American Heart Association certification.
- Licensure in states where care is delivered.
- 1 year of experience in acute hospital care, infusion, or home health preferred.
