About the Role

We are seeking a skilled RN Infusion Liaison to join our team at Providence Home Services Oregon. As a key member of our infusion team, you will provide direct nursing services related to infusion therapy for patients in various settings, including hospitals, homes, clinics, and alternate living sites.

Key Responsibilities:

  • Provide education to patients discharging from area hospitals, focusing on infusion therapy and related care.
  • Collaborate with various providers, patients, caregivers, physicians, and payors to ensure seamless care coordination.
  • Deliver infusion nursing care in the home or clinic setting, working closely with patients and their families to ensure their needs are met.
  • Develop and maintain strong relationships with healthcare providers, payors, and community partners to promote Providence Home Services Oregon's infusion services.

Requirements:

  • Current Oregon Registered Nurse License.
  • National Provider BLS - American Heart Association certification within 30 days of hire.
  • Licensure in states where care is delivered upon hire.
  • 1 year of experience with acute hospital care, infusion, or home health preferred.
