Key Responsibilities:

  • Provide personalized physical therapy services to patients in their homes, developing and implementing tailored treatment plans to meet their unique needs.
  • Collaborate with licensed Physical Therapists to ensure seamless delivery of care and achieve optimal patient outcomes.
  • Conduct thorough assessments to identify patients' goals and develop effective interventions to address functional limitations and mobility issues.
  • Document patient progress and communicate regularly with multidisciplinary teams to ensure comprehensive care.
  • Stay up-to-date with current evidence-based practices and best treatments to deliver high-quality care.
Requirements

To be successful in this role, you will possess a strong foundation in physical therapy principles and practices, excellent communication skills, and a commitment to delivering compassionate patient-centered care. Preferred Qualifications:

  • PTA Certification for the state of TX
  • CPR Certification
  • Professional Liability Insurance
  • Home Health or Rehab experience preferred
