Company OverviewLivWell Community Health Services is a non-profit organization dedicated to delivering comprehensive health and support services, education, and advocacy concerning HIV/AIDS, sexually transmitted infections, and viral hepatitis. We aim to provide quality healthcare to individuals affected by these conditions while also offering support services to clients and their families. Our mission includes prevention education and testing for all clients and the broader community, with a focus on reducing stigma and promoting awareness and equality.
Role DescriptionThe Housing Support Coordinator acts as a vital link, facilitating and coordinating efforts to ensure effective operations and decision-making regarding housing requirements. This role is essential in promoting access to affordable housing services and resources, offering individualized support to guide clients through the social services and healthcare systems to achieve housing stability.
Key Responsibilities- Conduct comprehensive initial housing assessments with clients to determine eligibility for services and present available program opportunities.
- Deliver community-based housing support services tailored to meet the identified strengths-based housing objectives, including housing search, placement, and retention.
- Assist clients in completing housing assessments, rental applications, and other necessary documentation.
- Evaluate and provide the appropriate level of assistance based on housing assessments and address emergency needs as per policy.
- Coordinate client-centered services to develop initial care plans, including resource identification, transportation arrangements, education, advocacy, and connections to external agencies for immediate housing needs.
- Maintain accurate client files, input data, and document interactions appropriately.
- Collaborate with a multidisciplinary team to support client medication adherence and report housing status during initial engagement.
- Manage scheduling and documentation of emergency housing needs in close collaboration with assigned case managers.
- Ensure adherence to quality standards, departmental goals, and all relevant policies and procedures.
- Engage in other assigned duties, including occasional travel for training and client visits.
- Develop and maintain relationships with landlords, conduct training, and serve as an advocate between clients and landlords across service areas.
- Ensure program compliance through file reviews and other assigned reporting tasks.
- Work closely with case managers to maintain service delivery without gaps.
- Understand all responsibilities within the housing program to assist or back up functions as necessary.
- Establish and nurture relationships with community partners, landlords, and property management entities.
- Create a database to compile landlord information, unit availability, and housing resources relevant to service areas.
- Assist in completing rent reasonable forms and documentation.
- Actively pursue the retrieval of housing deposits when applicable.
- Complete exit documentation and landlord surveys as required.
- Perform any other duties as assigned.

- Bachelor's degree in social work, Human Services, or a related field.
- Two years of relevant social work experience in housing.
- Master's degree in social work, Human Services, or a related field.
- Four or more years of relevant social work experience in housing.
- Experience in supervisory roles or reporting.
