Under the supervision of the Social Work Manager or designated representative, the Social Services Coordinator will deliver essential social work services, which include: advocating, assessing, assisting, collaborating, educating, evaluating, implementing, and planning for clients in various dimensions of their lives. This encompasses psychological, psychosocial, financial, environmental, and interpersonal issues. Engagement will occur in multiple environments, including but not limited to clients' residences, community sites, and healthcare facilities.

The Coordinator will act as a vital link between the client/family, the Interdisciplinary Care Team, and other community organizations (such as acute care facilities, skilled nursing homes, and assisted living facilities) to ensure a high level of satisfaction throughout the care process. Responsibilities also include providing support to clients in adapting to illness and/or disability, conducting psychosocial evaluations, facilitating discharge planning, crisis intervention, mental status assessments, and offering assistance to terminally ill patients.

Qualifications:

Education: M.S.W. from an accredited institution
Experience: Minimum of one year in a healthcare environment. A vehicle is required for field visits.
License: Valid certification and licensure from New York State.
