Job Summary

Neighborhood House is seeking a skilled Case Aide to provide case management services to approximately 20-30 clients. The ideal candidate will have experience working with diverse populations and navigating complex Federal and State regulations.

Responsibilities
  • Maintain a caseload of 20-30 clients, completing screening and assessments, home visits, and other requirements to maintain program compliance.
  • Utilize DSHS and ADS data systems to generate reports and communicate with clients, case managers, and providers to ensure compliance with DSHS requirements.
  • Coordinate documentation, authorization, and ordering processes for medical equipment requests, developing relationships with providers to improve efficiency.
  • Attend trainings and professional development activities, preparing trainings for administrative and clinical teams as requested.
  • Provide language support to the team and translate flyers and proofread documents as assigned.
  • Perform various administrative tasks, including managing e-fax communication, handling mail correspondence, and monitoring phone calls.
Qualifications
  • Associate degree in social work, social sciences, health, or a related field, or equivalent experience.
  • Two years of experience working in social services, health, or a similar environment with an emphasis on diverse populations.
  • Demonstrated ability to perform job duties within complex Federal and State regulations and complete technical documentation.
  • Very organized with a high level of attention to detail and ability to provide supportive customer service in a high-paced environment.
  • Ability to communicate effectively with individuals from diverse cultural backgrounds and demonstrate a commitment to advancing racial equity and social justice.
