About the Role:
A prominent nonprofit organization is seeking a dedicated Case Manager to join their Transitional Housing team. This position plays a crucial role in supporting individuals facing homelessness, including those with mental health challenges and substance use disorders, as they work towards securing permanent housing.
Key Responsibilities:
The Case Manager will be responsible for:
- Delivering direct support to a caseload of around 15 clients, which includes performing essential assessments, assisting with benefits, and providing necessary services to help clients reach their housing and recovery objectives.
- Conducting comprehensive psychosocial evaluations.
- Linking clients to psychiatric care, medical services, and substance use treatment facilities as needed.
- Facilitating the acquisition of identification, public benefits, and other relevant income sources for clients.
- Meeting with clients weekly to evaluate their needs, promote harm reduction strategies, and formulate service plans.
- Preparing all necessary documentation for housing applications.
- Guiding and advising clients through the process of securing permanent housing.
- Maintaining accurate records in accordance with agency policies and the regulations of City and State agencies.
- Intervening in crisis situations and accompanying clients to appointments when necessary.
- Assisting clients with self-medication in line with the organization's protocols.
- Performing additional related duties as assigned.
Qualifications:
Applicants should possess:
- A Bachelor's Degree.
- Proficiency in computer applications.
- Strong skills in Microsoft Office Suite (Excel, Word, etc.).
- Excellent interpersonal abilities.
- Outstanding communication skills, both written and verbal.
- A keen attention to detail.
- Exceptional organizational skills.
Preferred Skills:
Ideal candidates will have:
- A minimum of 2 years of experience working with homeless individuals and addressing substance abuse issues.
- Familiarity with Harm Reduction and Motivational Interviewing techniques.
- Bilingual proficiency in Spanish.
