Job Summary

We are seeking a highly organized and detail-oriented Per Diem Temp AWV Coordinator to join our team at Southern New England Healthcare. As a Per Diem Temp AWV Coordinator, you will play a critical role in ensuring the timely scheduling and coordination of Annual Wellness Visits (AWVs) for our patients.

Key Responsibilities
  • Scheduling and Coordination: Schedule AWVs for patients based on their eligibility and availability, following HIPAA guidelines.
  • Patient Interaction: Communicate with patients to explain the purpose and benefits of the AWV, assist with completing necessary forms and documentation, and address any questions or concerns.
  • Documentation and Record Keeping: Maintain accurate and up-to-date records of patient AWV appointments and follow-ups, ensure all patient information is entered into the electronic health record (EHR) system accurately, and monitor and track AWV completion rates.
  • Compliance and Quality Assurance: Ensure all AWV procedures comply with CMS healthcare regulations and standards, review patient charts to ensure all required components of the AWV are completed, and communicate with responsible providers on incomplete information and/or areas of concern.
Requirements
  • Education: High school diploma or equivalent; additional education or training in healthcare administration or medical assisting is preferred.
  • Experience: Previous experience in a healthcare setting, particularly with patient scheduling or coordination, is beneficial.
  • Skills: Strong organizational and time-management skills, excellent communication and interpersonal skills, proficiency with Electronic Medical Record (EMR) systems, and proficiency with MS Office 365 Excel, Outlook, and Teams.
Work Environment

This is an in-person position working in various providers' offices in central CT and the towns surrounding Springfield. The office or clinic setting requires interaction with patients, healthcare providers, and administrative staff in a fast-paced environment with a focus on efficiency and patient care.

Duration

The position is temporary, with the length of employment determined by the specific needs of the organization, averaging 3-6 months.
