Clinic Triage Nurse (RN) Job Description

Central City Concern is seeking a skilled Clinic Triage Nurse (RN) to join our team. As a Clinic Triage Nurse, you will play a critical role in providing high-quality patient care and triage services in our outpatient clinics.

Key Responsibilities:
  • Assess and triage patients in-person and by phone, utilizing clinical decision-making skills to determine the level of care required.
  • Provide nurse visits, provider/nurse co-visits, and administer medications as ordered by providers.
  • Collaborate with interdisciplinary teams to provide comprehensive patient care and coordinate referrals.
  • Participate in quality improvement activities and contribute to the development of clinical best practices.
  • Support the integration of services at the Blackburn Center and identify opportunities for improvement within Blackburn Center workflows and logistics.
Requirements:
  • Current RN license in the State of Oregon, as issued by the Board of Nursing.
  • Successful completion of a course of accredited study that leads to the AD, or BS or AA degree in Nursing from OSBN or State Board approved course.
  • Experience with care for individuals with complex substance use and mental health backgrounds.
  • Ability to work independently, prioritize tasks, and maintain a calm professional demeanor in a fast-paced environment.
