Job Summary

We are seeking a skilled Registered Nurse to join our team at IntelyCare. As a Registered Nurse, you will provide expert nursing care to patients in post-acute care facilities, working closely with residents and medical staff to develop care plans and ensure the highest level of patient care.

Responsibilities
  • Provide expert nursing care to patients in post-acute care facilities
  • Develop and implement individualized care plans in collaboration with residents and medical staff
  • Monitor and track patient outcomes, making adjustments to care plans as needed
  • Collaborate with medical staff to ensure accurate and timely documentation of patient histories and interventions
  • Supervise and train nursing staff, ensuring compliance with facility policies and nursing standards
  • Participate in incident investigations and provide recommendations for improvement
Requirements
  • Current Connecticut state license to practice as a Registered Nurse (RN)
  • Graduation from an approved nursing program
  • At least 1 year of experience as a Registered Nurse or similar role
  • Excellent organizational and leadership skills
