Job Summary

We are seeking a skilled Licensed Practical Nurse to join our team. As a key member of our healthcare team, you will be responsible for providing high-quality patient care and ensuring the well-being of our clients.

Responsibilities
  • Provide individualized care to clients, following their personalized care plans and maintaining their dignity and independence.
  • Monitor clients' vital signs and report any abnormal findings to the case manager and physician.
  • Administer medications, feedings, oxygen, and other treatments as needed, and document the results.
  • Complete routines established by other care professionals, such as physical therapists, occupational therapists, and speech therapists.
  • Ensure infection control policies are maintained at all times and report any possible communicable diseases or infections.
  • Accurately document nursing actions and communicate with family, case managers, and physicians.
  • Order supplies and medications as needed to ensure adequate amounts are on hand.
  • Monitor incidents involving clients and ensure incident reports are completed timely and accurately.
  • Ensure all medical equipment is maintained and report any faulty equipment.
Requirements
  • Completed post-secondary coursework to facilitate licensure as a Licensed Practical Nurse or Licensed Vocational Nurse.
  • Knowledge of individual service plans, including understanding of client rights and health needs.
  • Proof of current license as a Licensed Practical Nurse or Licensed Vocational Nurse in good standing from the state(s) of practice.
  • CPR certification.
  • Excellent interpersonal skills, including customer service, patience, empathy, and compassion.
  • In-depth understanding of patient care policies and regulations.
  • Familiarity with symptoms and signs of improvement for several medical conditions.
