POSITION SUMMARY:

The Family Medicine Practitioner, under the guidance of the Physician Site Lead, delivers comprehensive medical and basic surgical care to patients.

This role encompasses evaluation, diagnosis, treatment, and thorough documentation.

Coordinates medical, surgical, ancillary, nursing, and other services to ensure optimal and cost-effective outcomes for patients in their care.

Develop treatment strategies that integrate therapy, medication, dietary modifications, or surgical interventions to address patient health issues and injuries. Monitor patient recovery and make referrals to specialists based on assessments of patient progress and conditions. This position offers the flexibility to work in an outpatient setting exclusively or a combination of inpatient and outpatient care.

WORK ENVIRONMENT:
The practitioner will be part of a dynamic outpatient primary care clinic that includes integrated behavioral health, clinical pharmacy, population health, and care management.

Family medicine is a specialty that provides ongoing, comprehensive healthcare for individuals and families, integrating biological, clinical, and behavioral sciences across all ages, genders, organ systems, and disease entities.

KEY RESPONSIBILITIES:

Clinical Responsibilities:

Exemplify excellence in the Family Practice domains of diagnosis, aligning treatment objectives with patient values, definitive treatment, supportive care, follow-up care, promoting wellness, and preventing illness.

Practice evidence-based Family Medicine within the scope of licensure, training, and credentialing. Collaborate with other physicians and healthcare providers to ensure optimal patient care and outcomes.

Work in partnership with other members of the Family Medicine care team, specialty care provider teams, and referring providers to deliver a world-class patient experience.

Provide a compassionate presence in service to others by attentively responding to the needs of patients/customers.

This includes introducing oneself and stating your purpose, inquiring about the patient's preferred name, spending quality time with the patient to understand their care goals, actively listening, and utilizing effective communication skills (appropriate touch, eye contact, etc.).

Prescribe medications in accordance with established policies and procedures for medication administration.

Ensure accurate documentation, coding, and billing for all patient care in the electronic health record or on the appropriate forms in a timely manner, adhering to defined standards.

Participate in clinic call duties and responsibilities.

Perform procedures within the scope of competencies and support the delivery, measurement, and enhancement of quality health measures as identified by the organization.

May undertake additional duties of similar complexity as required or assigned.
May review and approve chart notes for advanced practice providers as necessary.

Essential Functions:

Option to work in an outpatient-only capacity or a combination of inpatient/outpatient.

Be familiar with the organizational structure, communication systems, and billing processes, completing billing and coding documentation within assigned timelines.

Conduct all activities with the utmost professionalism and confidentiality.

Comply with all applicable laws, regulations, policies, and procedures, supporting the health system's corporate integrity efforts by acting ethically and appropriately, reporting known or suspected violations, and cooperating fully with all investigations and proceedings.

Maintain current and unrestricted licenses and certifications to uphold privileges at the health system's facilities, allowing for the performance of job duties.
Foster understanding and collaboration throughout the medical community.
Ensure compliance with the Medical Staff Bylaws, Rules, and Regulations as required by the governing board.

2 FTE of this position will be dedicated to residency participation in Family Medicine education:


The Associate Residency Director of Education is responsible for developing, implementing, and revising core residency curriculum in collaboration with the Residency Director and leadership team.

This role includes focused leadership in areas such as Resident Didactics, Clinical Experiences, Longitudinal Curriculum, Areas of Concentration, Capstone Program, and Elective support.

This individual will represent the residency in various graduate medical education meetings/work groups as directed.

They will be a member of the Clinical Competency Committee and prepare evaluations of residents to assess readiness for promotion and graduation.

This role involves collaboration with staff and faculty to align with educational goals and priorities.

Responsibilities include resident recruitment, onboarding, precepting, advising, performance reviews, and evaluations, along with participation in presentations, rotational oversight, teaching, and quality improvement initiatives.


This position reports to the Residency Director and is expected to work collaboratively with other members of the leadership team.

An administrative team provides support for all aspects of the Residency Program and faculty roles.

Duties/Functions:
This role will encompass a mix of the following areas in partnership with the Program Director and leadership team:
Didactic oversight
Coordinate Longitudinal Curriculum in alignment with program requirements.
Coordinate clinical didactics
Oversee Orientation curriculum
Recruit faculty leads in specific content areas and collaborate on curriculum delivery
Continuously evaluate and assess curriculum for effectiveness and relevance based on evolving Family Medicine practices and teaching best practices
Clinical Rotations
Provide oversight for required clinical experiences, including developing new rotations and liaising with rotation leads
Liaison with external services to address resident performance concerns
Evaluations
Oversee the evaluation process, including creating new evaluation forms and participating in the Clinical Competency Committee
Assist with preparation of summative evaluations
Advising
Support the resident advising process that integrates clinic-based and academic advising
Participate in academic advising sessions and summative evaluations
Provide faculty development on effective advising sessions and feedback
Electives
Establish relationships with providers to develop new focus areas based on resident needs
Quality Improvement
Coordinate Evidence-Based Answers and Improvement Science Curriculum, liaising with faculty leads in those areas
Overall Program Administration

Mentor faculty in the creation and delivery of didactics, simulations, workshops, evaluating residents, and providing feedback.

Be accessible to residents both in person and virtually.
Significant involvement in resident recruitment and selection processes, orientation activities.
