Responsibilities:

  • Assess available patient information and analyze its relevance to the evaluation and treatment strategy.
  • Perform and document a comprehensive functional assessment of the patient, employing standardized methods. Evaluate home, vocational, and equipment requirements.
  • Create and execute a personalized treatment plan tailored to patient needs, incorporating attainable functional objectives.
  • Adhere to evaluation, treatment, and documentation protocols while consistently maintaining accurate and timely departmental records.
  • Record a progress note in the patient file after each treatment session.
  • Adjust treatment plans as necessary throughout the therapeutic process.
  • Summarize the treatment course in a clear and concise discharge report using a specified format.

Qualifications:

  • Bachelor's or Master's degree in Occupational Therapy.
  • Valid license/certification in the state of practice.
