Resident Care Coordinator Job Summary

We are seeking a highly skilled and compassionate Resident Care Coordinator to join our team at Senior Living Communities. As a key member of our Assisted Living and Memory Care team, you will be responsible for implementing standards of resident care on a 24-hour basis, overseeing the nursing aspects, and directing and evaluating the care provided by our staff.

Key Responsibilities:
  • Maintain accurate and up-to-date resident charts, including admission documents, care plans, and records of care.
  • Ensure compliance with all regulations from federal, state, and local agencies, and serve as a liaison between our campus and regulatory agencies.
  • Administer resident medications, assess responses, and make adjustments or recommendations as needed.
  • Participate in and lead staff meetings, implement nursing programs, and conduct daily resident rounds.
  • Participate in and attend all in-service training and education programs as scheduled.
Requirements:
  • Two- or Four-year college or university degree required.
  • Current state licensure as a Nurse or CNA/Med Tech certification required.
  • Two years' experience in a similar healthcare position required.
  • Five years of healthcare experience desired.

  • Technical Skills: Demonstrated current and ongoing competence in tasks assigned, indicating a specialized skill in this area above and beyond the average.
  • Communication: Ability to effectively and regularly transfer information to necessary parties to improve the quality of work and provide actionable, accurate data.
