Job Title: Care Manager - Assisted Living

Sunrise Senior Living is a leading provider of senior care services, and we are seeking a highly skilled and compassionate Care Manager to join our team.

Job Summary:

The Care Manager will be responsible for providing high-quality care and services to a consistent group of residents in our assisted living and reminiscence neighborhoods. This includes building meaningful relationships with residents and their families, providing assistance with activities of daily living, and participating in the development of individualized service plans.

Responsibilities:

  • Build meaningful relationships with residents and their families
  • Provide assistance with activities of daily living, such as bathing, dressing, and grooming
  • Participate in the development of individualized service plans and monthly updates
  • Responsible for a designated group of residents during the shift, ensuring their safety and well-being
  • Partner with the community team to ensure compliance with OSHA requirements and promotion of risk management programs and policies
  • Respond to the dining needs of residents and guests while maintaining Sunrise hospitality and service standards
  • Invite, encourage, and assist residents in life skills and other life-enriching activities as indicated on their individual profile

Qualifications:

  • Dedication to serving seniors with excellent customer service skills
  • Positive attitude and flexibility to perform various duties in service to residents
  • High school diploma or GED accepted, with certifications and licenses required in some states
  • Previous experience working with seniors preferred
  • Ability to make choices, decisions, and act in the best interest of residents
  • Competent in organizational and time management skills
  • Demonstrate good judgment, problem-solving, and decision-making skills
