As a Home Health Charge Nurse, you will be responsible for providing quality, clinical services to patients in various settings in the community. You will work collaboratively with the interdisciplinary team to develop and implement patient care plans that meet the unique needs of each patient.
Key Responsibilities:
- Develop and implement patient care plans that meet the unique needs of each patient.
- Provide skilled nursing care, including assessment, teaching, treatment, and documentation.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
- Manage patient schedules and adjust as needed to meet clinical care and program needs.
- Reconcile medications accurately and timely at admission, discharge, and transfer.
- Make referrals to other disciplines and community resources to maximize patient safety and outcomes.
Education/Degree Requirements:
- Graduate of an approved School of Nursing.
- Current Massachusetts license as Registered Nurse.
- Minimum of one-year recent nursing experience in acute or emergency setting required.
- Home Care experience a plus.
