Role Summary
Under the guidance of the Director of Residential Care, the Patient Care Assistant (PCA) delivers direct support and oversight to residents while monitoring and reporting on their conditions to the Individual Service Plan Team.
Key Responsibilities
- Deliver empathetic and secure care for all residents.
- Address clients' needs promptly, professionally, and courteously.
- Assist with care and supervision as outlined in each resident's Individual Service Plan, including monitoring vital signs and documenting any signs of illness.
- Identify when assistance or medical intervention is required and act accordingly.
- Track residents' vital signs, contact medical professionals, or arrange emergency transport as necessary.
- Adhere to all organizational policies and procedures. Participate in all staff meetings, training sessions, and workshops.
- Ensure residents take their medications as prescribed, monitor weight changes, and report significant fluctuations to the Registered Nurse.
- Complete end-of-shift reports detailing resident conditions and house activities for the incoming shift and management.
- Conduct regular rounds to monitor residents and the facility, ensuring safety and well-being.
- Maintain confidentiality regarding resident information at all times.
- Foster positive relationships with residents, staff, volunteers, and external representatives.
- Exhibit self-motivation and initiative in organizing and fulfilling job duties.
- Assist residents with housekeeping and laundry as needed, ensuring a tidy living environment.
- Answer phone calls in a professional manner, taking accurate messages and relaying them promptly.
This role requires the ability to walk, stand, bend, squat, climb, kneel, and twist frequently. The position involves lifting up to 50 lbs. Employees must be vaccinated against COVID-19 or possess an approved religious or medical exemption.
Qualifications
- High School diploma or equivalent.
- Certification as a Home Health Aide or Nursing Attendant.
- Understanding of bloodborne pathogens and universal precautions, including the use of protective equipment.
- Successful completion of a criminal background check.
- Current BCLS and Red Cross Certification.
- Bilingual abilities (English/Spanish) are preferred.
- Cultural sensitivity and a non-judgmental approach are essential.
- Awareness and understanding of issues related to homelessness and/or HIV/AIDS.
- Basic knowledge of the care requirements for individuals with HIV.
- Ability to engage with diverse populations in a compassionate and respectful manner.
- Proficiency in reading, writing, speaking, and understanding both English and Spanish.
- Certification in CPR/First Aid.
