Key Responsibilities:- Utilize the nursing process to identify and achieve patient goals, including assessment, planning, implementation, and evaluation.
- Assess the physical, psychosocial, and environmental factors that affect a patient's health to develop a comprehensive nursing care plan.
- Collaborate with the interdisciplinary team to ensure personal care, medical care, and rehabilitation provide optimal patient health and well-being.
- Teach patients and caregivers about various aspects of healthcare and disease management, documenting continuing needs and levels of understanding.
- Unencumbered current Registered Nurse license in the state where the HomeCare agency is located or compact state, if applicable.
- Minimum one (1) year experience as a RN/professional nurse.
- Experience in home care is desirable.
