About the Role

We are seeking a skilled and compassionate Registered Professional Nurse to provide high-quality, patient-centered care to individuals in their homes, promoting independence, dignity, and comfort.

Responsibilities
  • Assess patient needs and develop individualized care plans to meet their unique requirements.
  • Provide direct and indirect care to patients, including wound care, medication management, and education on self-care techniques.
  • Collaborate with the healthcare team to coordinate patient care activities and ensure seamless transitions between care settings.
  • Uphold the standards of confidentiality, attendance, and punctuality, maintaining a professional and respectful demeanor at all times.
Requirements
  • Education to meet certification, license, or registration requirements.
  • Oregon Registered Nurse License upon hire.
  • National Provider BLS - American Heart Association within 30 days of hire.
  • 1 year of medical/surgical experience in an acute care setting required for Home Health RN.
  • 2 years of Pediatric experience in an acute care setting for RN caring for pediatric patients.
