The City and County of San Francisco is seeking a highly skilled Clinical Pharmacist to join our team at the San Francisco Department of Public Health. As a Clinical Pharmacist, you will play a critical role in providing pharmaceutical care to patients and contributing to the development of medication use policies and guidelines.
Key Responsibilities- Interview and examine patients for drug evaluation, check on drug adherence and side-effects, and discuss symptoms and effects of prescribed drugs with patients and maintain a drug history on patients interviewed.
- Consult with physicians on drug usage and the drug regime of specific patients, particularly hypertensive, diabetic, and psychiatric patients, and recommend changes in the drug or the dosage prescribed for these patients.
- Conduct both formal and informal training for nursing staff, medical and pharmacy students and residents, and other healthcare staff on drug utilization, dosage, incompatibilities, preferred dosage forms, evidence-based medication therapy, and toxicity.
- Participate in the development and revision of medication use policies and guidelines.
- Compound drugs, prepare, and dispense medications, check pharmacy stock, and keep records.
- Requires possession of a valid Registered Pharmacist license issued by the California State Board of Pharmacy.
- One (1) of the following: completion of six (6) months of a pharmacy residency program, completion of six (6) months of a post-graduate pharmacy fellowship program, two (2) years of professional level experience as a Clinical Pharmacist, or possession of a Board Certification in a pharmacy specialty area.
- Acute Care practice: demonstrated experience with the acute care medicine patient population.
- Clinical and/or lived experience with diverse communities.
- Excellent oral and written communication and interpersonal skills.
- Ambulatory Care practice: completion of an ambulatory/primary care pharmacy residency, specialty residency, or fellowship.
- Fluency in Spanish and/or Cantonese is highly desired.
- Mission-driven, passionate candidates with Primary Care clinical expertise and lived experience with Black/African American, Latinx, LGBTQIA+, and other diverse and intersecting communities.
- Meets requirements for prescribing as required by Standardize Procedure for Ambulatory Care Pharmacist within San Francisco Health Network.
- Behavioral Health practice: completion of a psychiatric pharmacy residency.
- Clinical experience working in substance use disorder or mental health settings.
- Fluency in Spanish and/or Cantonese is highly desired.
- Mission-driven, passionate candidates with Mental Health/Behavioral Health/Substance Use clinical expertise and lived experience with Black/African American, Latinx, LGBTQIA+, and other diverse and intersecting communities.
- Board Certification as a Psychiatric Pharmacist.
- Possession of a valid Advanced Practice Pharmacist license in California.
- Long Term Care practice: completion of a geriatric pharmacy residency.
- Certification in Geriatric Pharmacy.
