We are seeking a dedicated and accomplished RN Hospice Specialty Registered Nurse to join our team in the Farmington, CT area.

About the Role:

The RN Hospice Nurse will ensure quality and safe delivery of hospice nursing services to patients in the home or assisted living setting.

Key Responsibilities:

  • Develop, implement, and evaluate the plan of care and make necessary revisions to address all problems in the plan of care.
  • Provide coordination of the plan of care, maintaining continuity of client care with other health professionals while identifying interdisciplinary care needs and making recommendations for additional home health care services.
  • Supervise the client care rendered by Licensed Practical Nurses and Home Health Aides in the provision of delegated duties and conduct supervisory visits per regulatory guidelines.
  • Provide education, supervision, and counseling to the client and their family regarding home nursing procedures and other care needs as appropriate to client's needs.
  • Assure that the client and their family are involved in the development of the plan of care whenever possible.

Requirements:

  • Current RN License required.
  • Hospice AND/OR Home Care experience IS PREFERRED.
  • NEW GRADS WILL NOT BE CONSIDERED.
