FellowshipLIFE is seeking a dedicated Physical Therapist to join our dynamic team. As a key member of our home-based care team, you will provide personalized therapy services to our residents in the comfort of their own homes.
Key Responsibilities:- Conduct thorough assessments of clients in their homes, focusing on mobility, strength, flexibility, and overall physical function.
- Develop personalized treatment plans based on home assessments, considering the unique needs and living environments of clients.
- Implement a range of therapeutic interventions, exercises, and modalities to improve clients' mobility, strength, and functional abilities within the comfort of their homes.
- Provide supervision to Physical Therapist Assistants, including case conferencing, co-signing treatment notes, and onsite client supervisory visits.
- Provide client education, ensuring a clear understanding of conditions and treatment plans, and offering guidance on home exercises for ongoing progress.
- Regularly reassess clients and modify treatment plans as needed to achieve optimal outcomes in a home-based setting.
- Utilize effective pain management techniques to enhance clients' overall well-being.
- Foster clear communication with clients, their families, and healthcare team members to provide personalized and effective home-based care.
- Active New Jersey Physical Therapist License.
- 2+ years preferred but new graduates are encouraged to apply.
- Valid New Jersey Driver's License.
- Demonstrate unwavering professionalism with clients, team members, and families.
- Exhibit strong interpersonal, verbal, and written communication skills.
- Showcase the ability to adeptly navigate as needed.
- Must travel throughout Union County to service clients.
