Job Summary:
We are searching for a highly motivated and compassionate Physical Therapist to provide high-quality care to our clients in the comfort of their homes and in assisted living and independent living settings.
Key Responsibilities:
- Conduct thorough assessments of clients in their homes, focusing on mobility, strength, flexibility, and overall physical function.
- Develop personalized treatment plans based on home assessments, considering the unique needs and living environments of clients.
- Implement a range of therapeutic interventions, exercises, and modalities to improve clients' mobility, strength, and functional abilities within the comfort of their homes.
- Provide supervision to Physical Therapist Assistants, including case conferencing, co-signing treatment notes, and onsite client supervisory visits.
- Offer client education, ensuring a clear understanding of conditions and treatment plans, and providing guidance on home exercises for ongoing progress.
- Regularly reassess clients and modify treatment plans as needed to achieve optimal outcomes in a home-based setting.
- Utilize effective pain management techniques to enhance clients' overall well-being.
