POSITION SUMMARY:
The LPN delivers personalized nursing care and comprehensive services to patients and their families in the comfort of their homes, under the guidance of a Registered Nurse.
KEY RESPONSIBILITIES:
The following responsibilities are typical for this role. This list is not exhaustive, and additional tasks may be assigned.
- Administers basic patient care under the supervision of a Registered Nurse, adhering to physician's directives.
- Monitors patients, documenting and reporting any changes in their conditions, including adverse reactions to medications or treatments, and taking appropriate actions.
- Provides education to patients and families regarding self-care and health management.
- Participates in case conferences as needed.
- Complies with organizational policies and procedures.
- Engages in quality management and safety initiatives within the agency.

Educational Background and Experience:
- Graduate from an accredited nursing program.
- Current licensure as a practical nurse in the state of Massachusetts.
- Current CPR certification.
- Minimum of two years of nursing experience.
- Valid MA driver's license and dependable transportation.
- Primarily works indoors in various climate-controlled environments.
- Potential exposure to unpleasant odors.
- Possible exposure to chemicals as outlined in the MSDS Manual.
- May encounter hostile or emotionally distressed family members, staff, and visitors.
- May require working beyond standard hours, including weekends and holidays as necessary.
- Travel to multiple work locations may be required, potentially covering up to 200 miles daily.
