POSITION SUMMARY:

The LPN delivers personalized nursing care and comprehensive services to patients and their families in the comfort of their homes, under the guidance of a Registered Nurse.

KEY RESPONSIBILITIES:

The following responsibilities are typical for this role. This list is not exhaustive, and additional tasks may be assigned.
  • Administers basic patient care under the supervision of a Registered Nurse, adhering to physician's directives.
  • Monitors patients, documenting and reporting any changes in their conditions, including adverse reactions to medications or treatments, and taking appropriate actions.
  • Provides education to patients and families regarding self-care and health management.
  • Participates in case conferences as needed.
  • Complies with organizational policies and procedures.
  • Engages in quality management and safety initiatives within the agency.

QUALIFICATIONS:

To excel in this role, candidates must be able to perform each essential duty effectively. The qualifications listed below represent the knowledge, skills, and abilities required. Reasonable accommodations may be made for individuals with disabilities to perform essential functions.

Educational Background and Experience:

  • Graduate from an accredited nursing program.
  • Current licensure as a practical nurse in the state of Massachusetts.
  • Current CPR certification.
  • Minimum of two years of nursing experience.
  • Valid MA driver's license and dependable transportation.

PHYSICAL DEMANDS/WORKING ENVIRONMENT:

  • Primarily works indoors in various climate-controlled environments.
  • Potential exposure to unpleasant odors.
  • Possible exposure to chemicals as outlined in the MSDS Manual.
  • May encounter hostile or emotionally distressed family members, staff, and visitors.
  • May require working beyond standard hours, including weekends and holidays as necessary.
  • Travel to multiple work locations may be required, potentially covering up to 200 miles daily.
