Job Summary

Eliot is seeking an experienced Licensed Practical Nurse (LPN) to coordinate residential services for adults with developmental disabilities and brain injuries in the communities of Burlington, Haverhill, and Framingham.

Responsibilities
  • Provide education and consultation to individuals and staff on medical issues, treatments, medications, wellness, and prevention.
  • Develop and implement individualized care plans to meet the unique needs of each person served.
  • Collaborate with community medical providers and VNA services to coordinate medical care for individuals.
  • Ensure compliance with DPH/MAP regulations and conduct regular audits to ensure program adherence.
  • Supervise and train nursing staff, including job performance evaluations and disciplinary actions as needed.
Requirements
  • 3 years of related experience in a residential or community setting.
  • Strong communication and interpersonal skills to establish supportive relationships with individuals and staff.
  • Ability to work independently and as part of a team to achieve program goals.
  • Commitment to person-centered, strengths-based approach to support individuals with developmental disabilities and brain injuries.
