About the Role

We are seeking a skilled and compassionate Hospice Registered Nurse to join our team at Affinity Care of Pennsylvania. As an On Call RN, you will play a critical role in providing high-quality patient care and support to our patients and their families.

Key Responsibilities
  • Respond to patient needs and provide urgent clinical and psychosocial interventions during after-hours periods.
  • Conduct thorough assessments of patients and their families, determining appropriate interventions and care plans.
  • Collaborate with the hospice team, including physicians, social workers, and other healthcare professionals, to ensure seamless care coordination.
  • Provide skilled nursing care, focusing on pain management and symptom control, and document patient care thoroughly and timely.
  • Communicate effectively with patients, families, and healthcare providers to ensure informed decision-making and optimal patient outcomes.
  • Participate in ongoing education and professional development to maintain expertise in hospice care and stay up-to-date on best practices.
Requirements
  • Current licensure as a Registered Nurse in the Commonwealth of Pennsylvania.
  • Minimum two years of experience in hospice, home care, hospital medical-surgical, or oncology nursing.
  • Excellent observation, verbal and written communication skills, problem-solving skills, basic computer skills, basic math skills, and nursing skills per competency checklist.
  • Ability to lift, position, and/or transfer patients, as well as lift supplies and equipment.
  • Visual acuity and hearing to perform required nursing skills.
