Job Summary

We are seeking a skilled and compassionate Registered Nurse to join our Weekend Hospice Team at St. Luke's Hospital. As a Weekend Hospice Nurse, you will provide high-quality patient care and support to individuals and their families in a home-based setting.

Key Responsibilities
  • Assess patient needs and develop individualized care plans in collaboration with the interdisciplinary team.
  • Provide comprehensive nursing care, including medication management, wound care, and symptom management.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care transitions.
  • Participate in bereavement support and counseling for patients and their loved ones.
  • Document patient progress and outcomes in a timely and accurate manner.
Requirements
  • Current professional nurse licensure to practice in PA.
  • Valid driver's license and reliable transportation.
  • Ability to work independently and as part of a team.
  • Excellent communication and interpersonal skills.
  • Knowledge of hospice care principles and practices.
