Hospice Nurse

We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team at Amedisys, a leading provider of home health, hospice, and high-acuity care. As a Hospice Nurse, you will have the opportunity to make a meaningful impact on the lives of patients and their families, providing much-needed care in the comfort of their own homes.
Responsibilities- Perform initial nursing assessments on new admissions and develop individualized patient care plans.
- Collaborate with the care team to ensure continuity of patient care and communicate patient progress to the clinical manager, physician, and care team.
- Record pain/symptom management changes/outcomes and educate families on patient condition, needs, and care.
- Make referrals to other hospice care disciplines as needed and perform on-call responsibilities and services to patients/families as assigned.
- Current RN license, specific to the state(s) you are assigned to work.
- One year of nursing experience preferred, or consideration for the RN new graduate program.
- Current CPR certification and valid driver's license, reliable transportation, and liability insurance.
- Attractive pay rate of $40.00-$50.00 hourly.
