Job Overview:
As a UAS Clinical Operations Coordinator, you will play a pivotal role in ensuring the seamless coordination of member assessments and care. Your primary responsibility will be to facilitate communication and scheduling between various stakeholders involved in member care.
Reporting Structure:
This position reports to the Managed Long Term Care Plan and collaborates closely with nursing staff and other departments engaged in member care.
Key Responsibilities:
- Assist in the organization of ongoing member visits, including UAS assessments for field assessors, while effectively tracking and rescheduling any canceled appointments.
- Meet daily scheduling targets.
- Provide comprehensive daily reports on all scheduled visits.
- Support the uploading of all UAS documentation into member files as required.
- Generate correspondence for members and manage mailings as necessary.
- Facilitate communication between field assessors and the multidisciplinary care team.
- Document daily communications and adhere to care coordination documentation standards, including the recording of issues, interventions, and outcomes.
- Act as a liaison and advocate for members and their families, ensuring effective communication with physicians and the care team regarding assessment scheduling.
- Engage in ongoing educational activities to enhance professional development.
- Perform other assigned tasks as needed.
Experience Requirements:
Essential:
- Exceptional written and verbal communication skills.
- Proficiency in computer applications.
- Strong ability to manage multiple tasks simultaneously.
Desirable:
- Experience in a managed care setting.
- Experience working with the elderly population.
Technical Skills:
Preferred:
Proficiency in Microsoft Office Suite, including Excel.
Compensation:
Annual salary range of $43,000 to $47,000.
