We are seeking a skilled Speech Language Pathologist to join our Home Health team. As a Speech Language Pathologist, you will play a vital role in evaluating, directing, and providing speech/language pathology services to patients in their homes or facilities.
Key Responsibilities- Evaluate and assess patients' speech, language, and swallowing disorders
- Develop and implement individualized treatment plans to address patients' needs
- Provide instruction and training to patients and their families on alternative communication systems and strategies
- Collaborate with healthcare teams to ensure comprehensive care and coordination
- Document patient progress and maintain accurate records
- Master's degree in Speech Language Pathology or related field
- Current licensure and certification in Speech Language Pathology
- Minimum of six months experience as a Speech Therapist or Speech Language Pathologist
- Home Health experience preferred
- Excellent communication and interpersonal skills
