Job Summary

A dedicated CST Team Leader is sought to provide clinical and administrative supervision to a team of professionals. The ideal candidate will have a Master's degree in a relevant field and at least four years of experience working with adults with a Serious Mental Illness (SMI). The Team Leader will be responsible for monitoring the physical health, clinical status, and response to treatment of team members, as well as implementing engagement strategies to minimize discharges due to drop out.

Key Responsibilities

  • Implement assertive engagement techniques to identify, locate, engage, and retain individuals who cycle in and out of intensive services.
  • Develop and implement well-thought-out engagement strategies to minimize discharges due to drop out.
  • Conduct comprehensive behavioral health assessments to identify individual strengths, needs, abilities, resources, and preferences.
  • Participate in transitional planning via in-person or teleconference meetings between stakeholders.
  • Ensure individuals have access to services and resources, such as housing, pharmacy, benefits, and a support network, upon discharge from a psychiatric hospital or release from jail.
  • Assist individuals with the application process to obtain entitlement benefits, including SSI/SSDI, Food Stamps, VA, Medicaid, and other relevant programs.

Requirements

  • Master's degree from an accredited university or college in a relevant field.
  • Minimum of four years of documented experience working with adults with a SMI.
  • Certified/credentialed as a substance use disorder counselor (CAC-I or equivalent or higher) is preferred.
  • Licensed clinician (LPC, LCSW, LMFT) is preferred.
