Occupational Therapist Opportunity

We are seeking a compassionate and skilled Occupational Therapist to join our team at Hope at Home Health Care. As a key member of our home health care team, you will provide high-quality occupational therapy services to patients in the comfort of their own homes.

Responsibilities:
  • Assess and evaluate patients' therapeutic, rehabilitative, and functional status
  • Develop and implement individualized plans of care to meet patients' needs
  • Provide occupational therapy services, including directing treatment and instructing patients and caregivers
  • Conduct home environment evaluations and assess for muscle strength, mobility, and range of motion
  • Identify patients' and caregivers' needs for other home health services and refer as necessary
  • Prepare and submit clinical and progress summaries based on patients' attainment of goals
  • Participate in discharge planning and provide occupational therapy consultation to patients and caregivers
Requirements:
  • Master's degree in Occupational Therapy from an accredited program
  • Current Occupational Therapy License in the state
  • Two years of experience as an Occupational Therapist, preferably in home health or community settings
  • Excellent communication and organizational skills
  • CPR certification
