Job Summary

We are seeking a compassionate and skilled Occupational Therapist to join our team at Hope At Home Health Care. As a key member of our home health team, you will provide high-quality occupational therapy services to patients in the comfort of their own homes.

Responsibilities:

  • Assess and evaluate patients' functional status and develop individualized plans of care.
  • Provide occupational therapy services, including assessing and evaluating therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care.
  • Direct and instruct patients and families/caregivers in the use and care of therapeutic appliances.
  • Identify patient and family/caregiver needs for other home health services and refer as necessary.
  • Prepare and submit clinical and progress summaries based on the attainment of goals.

Qualifications:

  • Possesses a degree from a program in Occupational therapy approved by an accredited organization.
  • Licensed to practice as an Occupational therapist within the state.
  • Two years of appropriate experience as an Occupational therapist, with community/home health experience preferred.
  • Demonstrates good verbal and written communication and organization skills.
  • Possesses and maintains current CPR certification.

Job Type: Part-time

Pay: $60.00 - $70.00 per Visit
