About the Role

We are seeking a highly skilled Admission Coordinator to join our team at Heartland Hospice. As a key member of our admissions team, you will be responsible for ensuring the timely and efficient admission of patients to our program.

Key Responsibilities
  • Contact patients and families within 1 hour of referral to coordinate admissions
  • Process referrals by obtaining necessary admission and insurance information
  • Participate in marketing efforts to develop and maintain positive relationships with referral sources
  • Ensure maximum reimbursement through initial payer source verification, benefits, and eligibility verification
  • Provide accurate insurance and admission information to admission staff
  • Collaborate with billing coordinator to ensure accurate billing documentation
Requirements
  • High school diploma or equivalent with customer service experience required
  • Strong communication, organization, negotiation, and public relation skills
  • Proficiency with PC and MS suite of products
  • Hospice or medical office experience preferred
