Nurse Care Coordinator Home Health

UPMC Home Health is seeking a skilled RN Patient Care Coordinator to join our team in the Shadyside, Squirrel Hill, Sharpsburg, Verona, and surrounding areas.
Job Summary:The Patient Care Coordinator is an on-site clinical team member responsible for coordinating care for assigned teams. This role involves daily triage and follow-up with referral sources, patients, clinicians, and clinical leaders to ensure seamless communication and care navigation.
Key Responsibilities:- Provide care navigation through daily triage and follow-up with referral sources, patients, clinicians, and clinical leaders.
- Assist the team at the Seven Fields location, working Monday to Friday daylight shifts, with weekend and holiday rotations.
- Participate in on-call coverage rotation for the agency.
- Oversee the ordering and approval of medical supplies to ensure accuracy and appropriateness.
- Cover for other Patient Care Coordinators when needed.
- Perform medication reconciliation on all therapy-only cases as needed.
- Monitor admissions, discharges, and re-hospitalizations, and communicate with clinical liaisons or facilities to facilitate continuity of care.
- Aid clinical staff in follow-up communication with MD offices and referral sources as applicable.
- Coordinate all incoming orders associated with census to ensure appropriateness and accuracy.
- Assist in document control processes, including review of Face-to-Face documentation and physician orders.
- Report problems immediately to the Clinical Manager.
- One year of recent home health experience preferred.
- Current licensure in the state where the facility is located or a multistate license issued by a participating NLC state.
- Strong problem-solving abilities, excellent telephone, interpersonal, and written communication skills.
- Strong/proven data entry and general computer skills.
- Ability to travel to assigned geographical locations as needed.
