Job Summary

We are seeking a skilled RN Care Coordinator to join our team at Effingham Health System. As a key member of our healthcare team, you will be responsible for coordinating care plans in compliance with regulatory requirements.

Key Responsibilities:

  • Develop and implement care plans in collaboration with medical staff and other departments.
  • Monitor and document resident assessments and care plans.
  • Ensure residents receive high-quality care in accordance with relevant guidelines and policies.
  • Communicate effectively with medical staff, other departments, and outside agencies while maintaining confidentiality.

Requirements:

  • Education: High school completion and completion of a minimum Practical Nurse Training course of study.
  • Formal Training: Management skills with experience in planning, organizing, implementing, facilitating, interviewing, counseling, and verbal and written communications.
  • Licensure, Certification, Registration: Unencumbered Georgia Licensed Practical Nurse license or Registered Nurse License and Basic Life Support CPR certification.
  • Work Experience: One to two years of related experience and/or training preferred.
