Nurse Care Coordinator

About the Role:
We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Level of Care Coordinator. In this role, you will be responsible for assessing the healthcare needs of individuals and developing comprehensive care plans to ensure optimal health outcomes.
Key Responsibilities:
- Conduct comprehensive assessments of individuals' healthcare needs, including medical, social, and emotional backgrounds.
- Develop and implement individualized care plans, taking into account the individual's level of need and functional ability.
- Collaborate with healthcare teams, including physicians, clinicians, and other support staff, to ensure seamless care coordination.
- Provide health education and instruction to individuals and their families on various health issues, including treatments, self-care, and preventive care.
- Advise clients on health maintenance and disease prevention, and provide case management services as needed.
- Communicate effectively with agency personnel, service providers, and other stakeholders to coordinate efforts and resolve operational problems.
Requirements:
- Current licensure as a Registered Nurse (RN) in the State of Georgia.
- Ability to work collaboratively with diverse stakeholders, including individuals, families, and healthcare teams.
- Strong communication and interpersonal skills, with the ability to effectively communicate complex healthcare information to individuals and families.
- Ability to work in a fast-paced environment, with multiple priorities and deadlines.
- Proficiency in electronic health records and other relevant software systems.
