Patient Care Manager RN

We are seeking a highly skilled and compassionate Patient Care Manager RN to join our team at SunCrest Home Health, a part of LHC Group. As a Patient Care Manager RN, you will play a vital role in ensuring the delivery of high-quality, patient-centered care to our patients in their homes.
Responsibilities:- Supervise and coordinate clinical services to ensure seamless care delivery.
- Provide clinical services within the scope of practice, as defined by state laws, in accordance with the plan of care and in coordination with other healthcare team members.
- Receive referrals, ensure appropriate clinician assignments, evaluate patient orders, and plot start of care visits.
- Coordinate determination of patient home health benefits, medical necessity, and ongoing insurance approvals.
- Ensure patient needs are continually assessed and care rendered is individualized to patient needs, appropriate and reasonable, meets home health eligibility criteria, and is in accordance with physician orders.
- Current RN licensure in the state of practice.
- Current CPR certification required.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
