Key Responsibilities
  • Actively participate as a member of the Interdisciplinary Team (IDT).
  • Oversee the coordination of the Plan of Care for assigned individuals, ensuring timely scheduling and facilitation of IDT meetings, along with the preparation and submission of necessary reports.
  • Communicate effectively with families, correspondents, and staff regarding IDT meeting schedules. Develop Individual Staff Action Plans during semi-annual meetings.
  • Address clinical issues promptly and efficiently.
  • Assist in training staff on the implementation of goals and services tailored for assigned individuals.
  • Maintain regular contact with appointed guardians and family members.
  • Evaluate and update service plans (DDP2, IPOP) as required, ensuring annual reviews are conducted.
  • Collaborate with care management to review and update Life Plans, ensuring accuracy and compliance.
  • Conduct site observations to ensure proper implementation of service plans and adherence to guidelines, including dining and behavior plans.
  • Prepare and oversee the execution of residential services.
  • Advocate for assigned individuals by identifying valued outcomes and personal goals.
  • Supervise service delivery in accordance with agency policies and state and federal regulations, participating in surveys as needed.
  • Collaborate with the agency's clinical department to develop behavior plans and guidelines for individuals on your caseload.
Qualifications
  • A Bachelor's Degree in a human services-related field, with a minimum of one (1) year of experience working with individuals with intellectual and developmental disabilities (QIDP status) and a valid NYS driver license.
  • Demonstrated ability to lead a team effectively, with strong verbal and written communication skills. Proven capability to coordinate comprehensive planning and implementation of individual program plans, alongside excellent organizational and problem-solving skills.
