Job Summary

This position is perfect for an individual who thrives in a collaborative environment and is committed to providing essential behavioral health care and social services to the community. The FACT Program operates as a multi-disciplinary clinical team responsible for delivering necessary treatment, rehabilitation, and support services to adults experiencing severe and persistent mental health challenges, fostering recovery, empowerment, self-determination, and informed decision-making for each participant.

Key responsibilities of the FACT team encompass delivering clinical and supportive services through advocacy, crisis intervention, assessments, evaluations, treatment planning, case management, and medication management. Participants receive community referrals, education on symptoms, vocational and educational support, and training in social and interpersonal skills, alongside assistance for their families and caregivers.

Most of a FACT team member's time is spent in the community executing these responsibilities, with only about 15% allocated to office work. While company vehicles are available, team members may also utilize their personal vehicles for service delivery and are eligible for mileage reimbursement.

The FACT team operates during and after standard business hours, with rotating holiday and weekend duties, and team members share the responsibility of carrying an emergency on-call cell phone.

Responsibilities
  • Deliver treatment, rehabilitation, and support services in alignment with the FACT model, based on the Program for Assertive Community Treatment (PACT) manual.
  • Ensure the accuracy, completeness, and confidentiality of clinical records.
  • Conduct mental health status assessments.
  • Coordinate referrals to community providers and support services.
  • Assist with activities of daily living, transportation, communication, and social skills.
  • Provide education regarding symptoms, medications, and mental health concerns.
  • Act as the primary case manager for a designated group of individuals, promoting recovery, empowerment, self-determination, and informed decision-making for each FACT participant.
Qualifications
  • A Bachelor's degree in Human Services or a related field is required.
  • Previous experience working with adults facing severe and persistent mental health issues (e.g., schizophrenia, bipolar disorder, borderline personality disorder) is essential.
  • Compliance with MHRC/RBHS vehicle driver requirements, including a valid Florida Driver's License, automobile insurance coverage of at least $200,000, and no more than eight points on the license for any combination of violations.
  • The role necessitates individual and group interactions with individuals experiencing serious and persistent mental health conditions in various environments.
  • Ability to make sound independent decisions daily, including recognizing emergencies and responding appropriately.
  • Strong communication skills are crucial, enabling effective interaction with internal and external stakeholders, including families, caregivers, community service providers, supervisory staff, and other professionals.
Position Details
Full Time Shift: Monday through Friday (with rotating weekends, on-call, and holiday responsibilities).
