Your primary responsibility will be to provide supportive housing services to clients, which may include:

  • Conducting assessments to identify client needs and creating personalized plans.
  • Connecting clients with relevant services, such as housing subsidies and Medicaid.
  • Developing and implementing strategies to promote social supports and community connections.

To be successful in this role, you will need:

  • A Bachelor's Degree in a relevant field, such as social work or psychology.
  • Experience in case management, preferably in supportive/transitional housing.
  • A strong understanding of social services and behavioral health principles.
