Job Title: Youth and Family YES Waiver Coordinator

Gulf Coast Center is seeking a dedicated and compassionate Youth and Family YES Waiver Coordinator to join our team. As a key member of our interdisciplinary team, you will provide critical support to children and adolescents enrolled in mental health services, ensuring they receive the necessary care and resources to thrive.

Job Summary:
  • Provide targeted case management, service coordination, transition planning, and crisis/safety planning for youth and families.
  • Implement the National Wraparound Initiative model for intensive case management, working closely with the participant, family, and chosen team to develop the participant's plan of care and recovery plan.
  • Assist individuals in obtaining YES Waiver Medicaid and provide targeted case management to facilitate the development of natural supports and assess treatment needs.
  • Collaborate with comprehensive waiver providers and HHSC to ensure participants receive appropriate, quality waiver services based on individual needs.
Responsibilities:
  • Manage assigned caseload, providing services authorized by a valid Recovery plan and CANS.
  • Conduct Children's Need & Strengths Assessments every three months or as clinically indicated, ensuring completion of initial and annual CANS recertification.
  • Display knowledge of TRR, ensuring proper protocol is followed during each intake scheduled.
  • Complete necessary consent forms, obtain pertinent records from external agencies, and complete QMHP assessments to determine level of care recommended for service delivery.
  • Establish initial person-centered recovery plans, with client and family input, indicating individualized problems, SMART goals, and SMART objectives following the TRR model.
Requirements:
  • Masters degree in behavioral sciences field of study from an accredited college or university.
  • Actively pursuing LPC-A or LMSW licensure, with ability to verify.
  • Preferred experience with children/adolescents, mental health, and leadership qualities.
Working Conditions:
  • Work will be performed in office and/or field settings, with potential for varied work hours and out-of-region travel.
  • Requires current Texas Driver's License, reliable transportation, and liability insurance.
