Job Summary

UC San Diego Health is seeking a highly skilled and compassionate Transplant Coordinator to join our Lung Acquisition team. As a key member of our multidisciplinary transplant team, you will provide clinical assistance and support in treating and managing the pre and post-transplant care of transplant patients.

Key Responsibilities
  • Provide clinical assistance and support in treating and managing the pre and post-transplant care of transplant patients.
  • Act as a key member of the multidisciplinary transplant team.
  • Solve complex clinical problems and communicate directly with the attending physician.
  • Consult and liaison for patients and their families with attending physicians, house staff, social workers, financial and administrative staff, and community physicians.
  • Provide teaching to patients, staff, and community organizations.
  • Collaborate with attending physicians on admissions, discharges, urgent evaluations and patient education, primarily in the hospital setting.
  • Work closely with the administrative team to fulfill QA and regulatory requirements.
Requirements
  • Must be a graduate from an accredited school of nursing.
  • RN License issued by the State of California.
  • BART or BLS certification at time of hire with commitment to get BART within six months of hire date.
  • Solid Organ Transplant Coordinator experience.
  • Ability to perform triage and handle critical and/or urgent situations with accuracy, expediency, and tact.
  • Strong critical thinking and problem-solving skills to manage multiple levels of information and responsibilities, and to quickly assess moderately-complex problems to develop potential solutions.
  • Must be able to function in a fast-paced environment, have excellent interpersonal and communication skills, and consistently demonstrate empathy and kindness to all staff and patients.
  • Demonstrates critical thinking and clinical skills to identify and implement nursing interventions and evaluate results.
  • Must have excellent skills to communicate and influence effectively with all levels of staff, physicians, patients and external constituents, both verbally and in writing.
Preferred Qualifications
  • BSN.
  • Certified Clinical Transplant Coordinator (CCTC) certification.
  • Prior Lung Transplant Experience.
  • Epic and Phoenix experience.
