Job Summary

We are seeking a highly skilled and experienced Registered Nurse to join our Transplant team at UC San Diego Health. As a Registered Nurse in our Transplant Clinic, you will play a critical role in providing exceptional patient care and contributing to the success of our team.

Key Responsibilities
  • Provide direct patient care to transplant patients, including assessment, planning, implementation, and evaluation of care.
  • Collaborate with the interdisciplinary team to develop and implement individualized care plans.
  • Administer medications and treatments as prescribed by physicians.
  • Monitor patients' vital signs and report any changes or concerns to the healthcare team.
  • Participate in patient education and counseling to promote health and wellness.
  • Document patient information and progress in the electronic medical record.
Requirements
  • Current RN licensure in the State of California.
  • Minimum of three years of recent RN experience in acute or ambulatory care.
  • Minimum of two years of recent experience working in an Ambulatory setting.
  • Previous experience working in an outpatient clinic, urgent care, or emergency department.
  • Proven independent thinking, decision-making, problem-solving, and prioritizing skills.
  • Proven experience of strong clinical and critical thinking skills.
  • Excellent communication and interpersonal skills.
Preferred Qualifications
  • BSN degree.
  • EPIC experience.
  • Previous experience working with solid organ transplant patients in an ambulatory setting.
  • Charge RN or other leadership experience.
  • Emergency department, Critical Care, or step-down experience.

Independent judgment is expected to be exercised within the context of interdisciplinary and multidisciplinary approaches to patient care. Job duties to include, but not limited to, telephone triage, RX refills, patient education, coordination of back office flow, phlebotomy, rooming patients, and medication administration. Will be required to schedule procedures and appointments as needed.
