About the Role

As a Home Health Registered Nurse with CenterWell Home Health, you will play a vital role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes. Our team is dedicated to providing comprehensive home care services that address the unique needs of each patient, promoting independence, and improving overall health outcomes.

Key Responsibilities
  • Provide skilled nursing visits to patients in their homes, assessing their needs, and developing personalized care plans to address their medical, functional, and emotional needs.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to ensure comprehensive care and coordination.
  • Administer medications, perform treatments, and provide education to patients and their families on disease management, self-care, and health promotion.
  • Conduct ongoing assessments and evaluations to ensure the effectiveness of care plans and make adjustments as needed.
  • Communicate effectively with patients, families, and healthcare providers to ensure seamless care transitions and coordination.
  • Document patient information accurately and thoroughly, maintaining confidentiality and adhering to regulatory requirements.
Requirements
  • Diploma, Associate, or Bachelor's degree in Nursing.
  • Minimum one year of nursing experience, preferably in home health or a related field.
  • Current and unrestricted Registered Nurse licensure.
  • Current CPR certification.
  • Strong organizational, communication, and interpersonal skills.
  • Valid driver's license, auto insurance, and reliable transportation.
