Job Overview

About the Role

This Clinical Care Coordinator position involves providing high-quality practical nursing care to individuals with mental or physical injuries, illnesses, or developmental disabilities. You will be responsible for implementing nursing care plans, performing various practical nursing and individual/patient/resident/inmate care duties, and observing and reporting changes in behavior.

Key Responsibilities
  • Prepare, administer, and record medications and treatments prescribed by physicians.
  • Observe and report changes in the physical and emotional behavior of individuals/patients/residents/inmates.
  • Take and record vital signs, collect specimens, and prepare progress notes and administrative reports.
Requirements

To be successful in this role, you must have a current Pennsylvania License for Practical Nurse and proficient computer skills, including Microsoft Office products. You should also be able to enter health data into electronic medical records databases.
